Progress Note, Treatment Plan, Conclusion Summary

The Progress Note, Treatment Plan and Conclusion Summary use the same format. That's because we need to be aware of the same group of issues in every session, even though we don't talk about each one every time. There's a saying, "If you don't know where you are going you are not going to get there. The treatment plan tells the direction where we are going. At the conclusion of our work together, we'll write a summary of where we started and where we arrived (it is usually called a Discharge Summary which sounds cold to me.)
